Hi, I really want to know if is possible to underline text in Sumatra PDF.

is correct that feature is not officially available
however if it is essential if “its possible” and you want to play you could try the SMX hack mentioned in
Hightlighting text (archive.org) HOWEVER smx is not available or necessary in current pre-release as normal underline annotation is now currently working.
